Transitions in Care Over the Lifespan

This on-demand learning activity includes three presentations covering three major care transitions in the lives of people with genetic disorders. The series will cover transition to a new diagnosis, the transition of a patient from pediatric to adult healthcare, and the transitions that can occur when providing care for children with life limiting conditions.

The learning opportunity is open to anyone in the healthcare community.
